HOW TO MAKE YOUR FIRST APPOINTMENT

Call Contra Costa County Access at (888) 678-7277 to request authorization number to Pathways to Wellness. Access will provide an authorization number at the time of the call. You should then contact Pathways To Wellness’s Martinez Intake Department at (925) 228-4200 for an appointment.

APPOINTMENTS
Your first appointment will consist of an hour of comprehensive intake/assessment with our psychiatrist and mental health specialist. Information gathered from this process will allow us to determine medical necessity, provide you with appropriate medications, and design a quality treatment plan.

Please bring any relevant materials (i.e. current medications, insurance card) to your first visit.

ELIGIBILITY
Pathways To Wellness Martinez only accepts Medi-Cal beneficiaries. For eligibility requirements at other Pathways to Wellness clinics please click here.
