Thank you for choosing Pathways to Wellness! Here’s what you can expect during your first visit to our clinic.
PATIENT PSYCHIATRIC EVALUATION

NEW PATIENT PSYCHIATRIC EVALUATION VISIT

For your initial visit, the first step will be to sign in at the front desk. After signing in, please provide the following:

  • Completed and signed enrollment forms
  • A copy of your insurance card
  • A valid photo ID
  • Current medication bottles or a detailed list of all medications you are taking
  • Any recent lab results, imaging studies, or psychological reports

What to Expect

Once we’ve received your signed forms and documentation, your vital signs will be taken. During your evaluation, your assigned medical provider will take the time to get to know you, discuss your background, and understand the reasons you are seeking psychiatric care. Most importantly, we want to know what you hope to achieve through our services. Our medical staff is committed to understanding both your past and present experiences to provide personalized care. 

While we will review any past diagnoses from previous providers, your new Pathways Medical Provider will conduct an independent assessment and provide their own clinical conclusions based on your history and the symptoms you share during the evaluation. Please allow 1 to 1½ hours for your first appointment. 

For children and adolescents (ages 5-17), a parent or legal guardian must be present for all appointments. 

MEDICATION MANAGEMENT FOLLOW-UP VISIT

Following your Initial Evaluation service, your medical provider will ask that the front desk staff schedule you a routine follow-up visit, known as a “medication management” visit. During these visits, your provider will: 

  • Discuss how your medications are working for you
  • Inquire about current life stressors and how you’re managing them
  • Provide medication refills or suggest changes, along with stress-reduction strategies when appropriate

Important Notice

Please note that the clinic does not provide phone refills without a medication management visit. It is crucial that you attend all scheduled appointments to ensure you have adequate medication. If you are unable to make a scheduled appointment, please contact the clinic immediately to cancel and reschedule so that you do not run out of medication.

THERAPY SERVICES

At our clinic, we offer solution-focused individual, group, and family therapy. Each therapy session is tailored with a specific focus, defined goals, and an estimated timeframe for completion. Your therapist will also determine the number of sessions required based on your needs. Sessions typically range from 30 to 90 minutes. 

You can request a referral for any of our therapy services from your physician or insurance carrier. 

HOW TO MAKE YOUR FIRST APPOINTMENT

For Medi-Cal and Medi-Cal/Medicare (Medi-Medi) Insured:

  1. Call Alameda County ACCESS: Dial (800) 491-9099 to request a referral to Pathways to Wellness.
  2. Referral Processing: Alameda County ACCESS will fax a referral letter to our Intake Department within 24-48 hours.
  3. Schedule Your Appointment: Once you have the referral, call us at (800) 975-PATH (7284) to schedule your first appointment.

Please note that we need authorization for services from Alameda County before any Medi-Cal-insured patient can be seen in our clinics.
