HOW TO MAKE YOUR FIRST APPOINTMENT
FOR MEDI-CAL Insured:
As a new client, you must call Alameda County ACCESS at (800) 491-9099 for a referral to Pathways to Wellness. ACCESS then faxes a referral letter within the next 24-48 hours to our Intake Department. Call us to schedule your first appointment, (800) 975-PATH (7284)

APPOINTMENTS
Your first appointment will consist of an hour of comprehensive intake/assessment with our psychiatrist and mental health specialist. Information gathered from this process will allow us to determine medical necessity, provide you with appropriate medications, and design a quality treatment plan.
Please bring any relevant materials (i.e. current medications, insurance card) to your first visit.
ELIGIBILITY
Our medical group contracts with Alameda County, PPOs and HMOs to provide behavioral healthcare services. Pathways to Wellness–Union City also accepts private pay clients. For Alameda County Medi-Cal beneficiaries, referrals come directly through the ACCESS system.
