Our Mission

Our mission is to provide a Pathway to Wellness for our community, by providing mental health services that are client-centered, and treat the whole person. At Pathways to Wellness we are passionately focused on the individual needs, concerns and strengths of our clients within and outside of the treatment setting where we meet — since our treatment modality is strength-based and recovery-oriented, our client’s autonomy and choices are always prioritized and honored. Treatment plans incorporate the available client and community resources to create a plan that really works, because it is sustainable.

At Pathways to Wellness, our mental health services structure is designed to provide holistic and compassionate care, ensuring that our clients receive the support they need to achieve and maintain mental wellness, and unlock their full potential. We offer a wide range of mental health care, including psychiatric evaluations, medication management, counseling, case management, and crisis care.

Welcome! We’re honored that you’ve chosen us to walk with you on your Pathway to Wellness! Here is what you can expect during your visits to the clinic.
PSYCHIATRIC EVALUATION VISIT

NEW PATIENT PSYCHIATRIC EVALUATION VISIT:

All new patients are required to bring completed enrollment forms, a copy of your insurance card and picture identification to your first visit. We also ask new patients to bring current pill bottles or a complete list of all medications that are currently being taken, a copy of any recent labs or imaging studies and any psychological reports to your initial visit. Your physician or nurse practitioner will give you time to introduce yourself, ask about your background, why you are seeking psychiatric assistance and most importantly find out what you wish to achieve with the help of our services.

Insurances

Pathways to Wellness accepts most commercial insurances and private pay patients. We also have contracts with Alameda County. Alameda County Medi-Cal beneficiary referrals come directly through the County’s ACCESS Department. Pathways to Wellness must obtain an authorization from ACCESS in order to provide any services to Medi-Cal beneficiaries.
